NCCDP Application Request for Certification for Memory Care Neighborhood Commendation For Assisted Living Communities and Nursing Homes

In order for a company to earn the Certified Memory Care Neighborhood Commendation credential, the company must meet the specified general criteria outlined in the application, the detailed specifications in the worksheet criteria, and pass the onsite visit by the NCCDP evaluator.
 

There are three steps; application process, worksheet criteria, and an onsite visit by the NCCDP evaluator.

Step 1: Download and complete the MCNC application and evaluation. Mail the completed application packet, evaluation, and payment to NCCDP. Once the NCCDP has received your application, the NCCDP will schedule a conference call with your organization to review the application. If after this discussion the NCCDP approves your application, you will be ready to move forward with the worksheet criteria.

Step 2: The worksheet criteria binder will be sent via FedEx ground to your organization. The organization designated team leader will have six months to complete the worksheets criteria. Once you have completed the worksheet criteria, mail the worksheet binder with supporting documents back to the NCCDP. Once the worksheet criteria are received and reviewed by the NCCDP evaluator, a conference call will be scheduled to review the worksheet criteria notebook. During this call, an onsite visit will be scheduled. The onsite visit will take place at your community by the NCCDP evaluator.

Step 3: The onsite visit. During the onsite visit, if everything is in order a certification will be issued to your team. Certified Memory Care Neighborhood Commendation. If everything is not in order, you will be granted an extension and another onsite visit will be scheduled. MCNC certification is renewed annually.

The Memory Care Neighborhood Commendation Certification indicates that the facility in question has complied with the criteria set by the National Council of Certified Dementia Practitioners for such Certification.  The certification is based on information provided by the facility and a one time visit by NCCDP.  At most, the Certification indicates that the facility appeared to be in compliance with NCCDP’s criteria for Memory Care Neighborhood Commendation  Certification on the date of the inspection.  There is a renewal process but no site inspection occurs unless the renewal application suggests a need for an on-site inspection.  Accordingly, NCCDP disclaims all liability for and knowledge of the compliance of the facility with NCCDP requirements after the date of such a visit by NCCDP to the facility.
